First Visit

When you come for your appointment please remember to obtain and bring the following:

• Insurance information (Authorisation Code and Policy Number)
• Referral Letter (if required)
• Copies of results, X-rays, MRIs, CT scans etc. and any other relevant information
• List of medications (if any)
• Please inform the hospital of any investigation you have had from the same condition or please let my secretary (Lorraine ) know
